Purpose

To obtain voice services (Telephone) on campus please submit a Telephone Services Request form to authorize charges to your department’s account. Then call the IT Help Desk at ext. 2020 (956-381-2020) to log your request. Once your request is logged Telephone Services may contact you for more information.

Scope

This SOP is intended for faculty and staff of The University of Texas Pan American in Edinburg Texas.

Prerequisites

1. Log your service request via the IT Help Desk at ext. 2020
2. You will need to submit a completed Telephone Service request form. The form may be obtained from URL: http://dba.panam.edu/forms/index.html#IT_index.  

Responsibilities

Request forms should be submitted to Telephone Services at ASB 1.102. We can be reached at ext. 7155 (956-316-7155)

Procedure

• Call the IT Help Desk at ext. 2020 (956-381-2020) to log your service request.
• Your service request will be assigned a Call ID number so that you can track your request.
• Download the Telephone Services Request form; complete; and submit to Telephone Services CO: Office Manager ASB 1.102.
• Please provide your Call ID number on your submitted Telephone Services Request form.
